From the dramatised accident that occurred in class, prepare an accident report according to the outline below

Preparing an Accident Report

Every accident must be reported to an employer (The Principal) and minor accidents reported to a supervisor (Your Teacher), safety officer or first aider and the details of the accident and treatment given suitably documented. The following information should be in the report:

1. The name and details of the person injured
2. Date and time of accident
3. A summary of what happened
4. A summary of events prior to the accident
5. Information about the injury or loss sustained.
6. Remedies employed
7. Details of witnesses (Name, age, address, position, etc)
8. Name of the person reporting the incident
9. Name and position of the person to whom the report is made

In addition to recording the above information:

• Sketch diagrams of how the accident occurred, where objects were before and after the accident, where the victim fell, etc.
• Take photographs or video that show how things were after the accident, for example, broken stepladders, damaged equipment, etc.
• Collect statements from witnesses. Ask them to write down what they saw. (Handwriting preferred, not typed)
• Record the circumstances surrounding the accident. Was the injured person working alone – in the dark – in some other adverse situation or condition – was PPE being worn – was PPE recommended in that area?
